We provide OOSA Membership Certificates to our members on request. The certificate will be sent to you in PDF format for you to print out.
Request a certificate
To order a membership certificate, please complete the form below with your details.
- Name: Enter the contact name that you have registered with us.
- Email: Enter the email address that you have registered with us.
- Certificate name: Enter the name as you wish it to appear on the membership certificate. This can be your own name (eg Jane Smith) or the name of your club (eg Fun Time Club).
- Membership number: You can check your membership number by clicking on the My Account link at the top of the page.
